Application for VA Education Benefits

When to use: If you have not previously applied for benefits through the VA VONAPP system

Purpose of the form: Apply for benefit eligibility certification

Directions: Complete one of these forms, as applicable

  • Chapter 30, 33, 1606 & 1607: Form 22-1990
  • Chapter 35: Form 22-5490
  • Chapter 31: Eligibility and application form is determined by the Department of Veteran Affairs, Vocational Rehabilitation Office

Request for Change of Program or Place of Training

When to use: If you have been using VA benefits at another school and are transferring to ASU

Purpose of the form: Transferring VA benefits to ASU from another school or university

Directions: Complete one of these forms, as applicable

VA Benefits Responsibility Form

When to use: Once, prior to the first semester of enrollment at ASU

Purpose of the form: Notifies ASU's Pat Tillman Veterans Center of your intent to use VA education benefits, such as the GI-Bill®.

Directions: This online form needs to be submitted only once, but please note that it contains important information regarding the use of VA benefits throughout your school enrollment. Complete the online form — it will submit directly to the Pat Tillman Veterans Center. You will receive a confirmation email indicating you have submitted the form.

VA Enrollment Activity Form

When to use: Before each semester, after registering for classes

Purpose of the form: Notifies the Pat Tillman Veterans Center if you do or do not want to use VA education benefits for the upcoming semester. When you elect to use your benefits, your enrollment information will be sent to the VA and ASU Student Business Services will place a tuition deferment on your student account for that semester. 

Directions: Fill out the form online. The form will submit directly to the Pat Tillman Veterans Center and you'll receive an email confirming you have submitted the form. 

Please make every effort to finalize your schedule before submitting the form. To notify us of changes you made to your enrollment after submitting the form for any given semester, submit the form again as a revised Enrollment Activity Form by following the choices within the form.

Prior Credit Evaluation

Prior Credit Evaluation

When to use: During the application process to ASU if transferring courses, credits and/or previous experience OR at the time of a change of major

Purpose of the form: Ensures that all prior credits are reviewed and appropriately applied to the student’s major and degree program

Directions: Print the form and fill out the student portion. Work with your academic advisor to complete the credits section. Submit the form to the Pat Tillman Veterans Center when it is completed.
